Information to the Applicant

As part of Reach Out, Inc.’s procedure for processing employment applications, personal and employment references may be checked. Applicants may make a written request for information derived from the checking of references. As necessary for employment, applicants are required to supply a birth certificate or other proof of authorization to work in the US, have a physical examination and/or drug test, or be entered into a pool for random drug testing while employed. I understand that there is a $19.00 fee for the OSBI report that will be run as part of the application process. I further understand that these fees will be deducted from my first paycheck. I also understand that should my employment last less than one year, the fees incurred for my training (i.e., MAT, CPR/First Aid) will be deducted from my final paycheck.

I understand that there will be a $50 fee for any class I am enrolled in and do not attend or give at least 24 hours’ notice prior to the class.

I understand that this application is good for only sixty (60) days from today’s date. If I still desire a position with Reach Out, Inc. after the expiration of the 60 days from the date of this application, it will be my duty to fill out another application and file it with Reach Out, Inc. Otherwise, I understand the company will not consider me for employment. I understand that if my application is considered favorable, I will be required to produce verification that I meet the necessary age requirements of the job I applied for and verify that I am legally entitled to work in the US before I begin employment.

I certify that the information in this application is correct to the best of my knowledge and understand that falsification or omissions in this application in any detail are grounds for disqualification or for dismissal from employment. I agree to conform to the rules and regulations of the company and understand that if hired, I will be an “at-will” employee and that my employment and compensation can be terminated with or without cause and with or without notice at any time at the option of either the company or myself.
